Print and complete form. Send Check with 40% Deposit and this signed document to
reserve the lot. Balance of your rent is due 1 week preceding arrival. A
Cancellation Notice of 30 days or more prior to arrival is required.
Your prepayment will be refunded less a 20% processing fee (minimum
$75.00). Rentals for six months or less are subject to a 6% Florida
State sales tax and 4% County Tourist tax. One (1) pet per lot (pets must be kept on a leash and cleaned up after). No refunds will be
given for early departure.

I -75 take exit
111 and turn east on Immokalee Rd (route 846). Travel approximately 4
miles to Collier Blvd, SR 951 and turn south. Crystal Lake RV
Subdivision Resort is 1/4 mile on the left, at the first traffic light.
Renter agrees for themselves and on behalf of all
persons who may use this property that they will abide by all of the
restrictions contained in the Homeowners Association documents including
the by-laws and Rules and Regulations. Renter also agrees that they are
responsible for any and all damages to said property during their rental
period. Renter agrees they will not trim any trees, bushes or foliage on
lot 4048. Violators will be subject to replacement costs of damages
incurred.
